This checklist includes practice indicators for establishing cohesive geographically-based teams.
Using a Primary Service Provider
This checklist includes indicators for selecting a PSP for a specific child and family as well as documenting the presence of coaching by the PSP with family members, care providers, and other team members.
Coordinating Joint Visits
This checklist includes indicators for implementing joint visits between the PSP and other team members with the family or other care provider.
