ASQ®-3 and ASQ®:SE-2 Trainer’s Handbook and USB Drive

Terms of Use

You, as an attendee who has completed the Ages & Stages Questionnaires® (ASQ®-3) and Ages & Stages Questionnaires®: Social-Emotional (ASQ®:SE-2) Training of Trainers, are granted permission to use the materials contained in the binder and accompanying USB drive only for the limited, non-commercial introductory training of your staff, colleagues, employees, or other members of your organization or program. If your organization or program has multiple sites or offices, you may use the materials to train staff, colleagues, and employees affiliated with each of those sites.

The material is designed so you can use selected items to deliver introductory training on the ASQ-3, the ASQ:SE-2, or both. For the introductory training, you may:

• use the PowerPoint slides to make your presentation
• photocopy the materials for distribution to the attendees

However, you are not permitted to:

• present a Training of Trainers seminar
• charge attendees or your organization or program for the training, other than for the exact cost to photocopy the materials
• provide training outside of your organization
• email or electronically reproduce the materials
• post materials online
• remove from the materials any copyright or other proprietary items, for example, but not limited to, copyright lines and the ASQ trademarks and logos
• use the materials in a way contrary to the family-oriented philosophies of the ASQ-3 and ASQ:SE-2 developers

It is required that you have an original set of ASQ-3 and/or ASQ:SE-2 Questionnaires and the ASQ-3 and/or ASQ:SE-2 User’s Guide at your training. Please see the Photocopying Release that accompanies the Questionnaires and User’s Guides for the terms under which these items may be reproduced, distributed, and used.

The materials contained in the Trainer’s Handbook and on the USB drive are protected by applicable copyright and trademark law. You will see the copyright protection line at the bottom of each form.

Neither Brookes Publishing nor the ASQ-3 and ASQ:SE-2 developers provide any warranty or guarantee as to the accuracy, timeliness, performance, completeness, or suitability of the information and materials included in the binder and/or the flash drive for an introductory training or acknowledge that such information and materials may contain inaccuracies or errors. We expressly exclude liability for any such inaccuracies or errors to the fullest extent permitted by law.

Your use of any information or materials in the binder and on the USB drive is entirely at your own risk, for which neither Brookes Publishing nor the ASQ-3 and ASQ:SE-2 developers shall be liable. It shall be your own responsibility to ensure that any products, services, or information available on the binder and the flash drive meet your specific requirements.

By using the Trainer’s Handbook and the electronic materials stored on the USB drive, you are agreeing to be bound by these Terms of Use, all applicable laws and regulations, and agree that you are responsible for compliance with any applicable local laws. Unauthorized use beyond the privileges outlined in this document is prosecutable under federal law.
